Job Summary:

We are seeking a highly skilled and experienced PCN Clinician to join our team at Service Care Solutions. As a PCN Clinician, you will play a vital role in providing high-quality patient care and support to individuals in their own homes.

Key Responsibilities:

  • Assess and diagnose patients with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains, and headaches.
  • Provide expert care, including assessment and diagnostic skills, for patients within their own homes.
  • Use a medical model to assess, diagnose, and prescribe for acute medical conditions.
  • Employ a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.
  • Work autonomously within the community, using enhanced clinical assessment and treatment skills.

  • Fixed-term contract (14 months)
  • Full-time hours per week
