Job Summary:

We are seeking a highly skilled and experienced PCN Clinician to join our team at Service Care Solutions. As a PCN Clinician, you will work autonomously within the community, providing first point of contact for patients presenting with undifferentiated, undiagnosed problems.

Key Responsibilities:

  • Provide enhanced clinical assessment and treatment skills to patients in the community
  • Use a medical model to assess, diagnose, and prescribe for acute medical conditions
  • Employ a holistic approach to consider methods and interventions to prevent recurrence of problems
  • Complete referrals to appropriate members of the wider multidisciplinary team

Requirements:

  • Advanced clinical skills and experience in community healthcare
  • Ability to work autonomously and as part of a team
  • Excellent communication and interpersonal skills
