Job Summary

We are seeking a highly skilled and motivated PCN Clinician to join our team at Service Care Solutions. As a PCN Clinician, you will work autonomously within the community, providing high-quality patient care and support.

Key Responsibilities
  • Provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains, and headaches.
  • Use a medical model to assess, diagnose, and prescribe for acute medical conditions.
  • Employ a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.
  • Work collaboratively with other healthcare professionals to provide comprehensive patient care.
