Position Overview

As a result of our expanding operations, we are excited to announce openings for Senior Clinical Nurse Practitioners.

The primary objective of this role is to function as an independent advanced practitioner, delivering exceptional and prompt patient-focused care.

The successful candidate will take charge of the evaluation, diagnosis, treatment, referral, and discharge processes for a wide variety of patients experiencing minor injuries or illnesses, all while adhering to their level of expertise and established protocols.

This position is designed to align with the responsibilities of a conventional in-hours Senior Clinical Nurse Practitioner within General Practice, distinctly differing from the Out of Hours Nurse Practitioner role.

Key Responsibilities

  • Operate as an advanced practitioner, showcasing advanced clinical skills and a knowledge base that surpasses traditional nursing roles.
  • Manage pre-scheduled nursing appointments akin to those in a standard GP Surgery.
  • Conduct patient triage.
  • Perform consultations, both in-person and via telephone, with patients across all age groups (from neonates to the elderly) as an independent practitioner, utilizing clinical judgment to diagnose, treat, refer, and/or discharge patients.
  • Facilitate direct referrals to primary, secondary, and social services in accordance with our established pathways, guidelines, and protocols.
  • Administer wound care and dressings.
  • Provide management for chronic diseases for pre-scheduled patients when assigned to qualified Nurse Practitioners.
  • Ensure that patients receive superior clinical care delivered promptly.
  • Prescribe, supply, and administer medications as necessary to meet patient needs.
  • Maintain thorough and accurate documentation.
  • Function both as an independent practitioner and as part of multidisciplinary and multi-agency teams to ensure patient needs are met.

When necessary, offer supervision and mentoring to support the development of other staff members' knowledge, skills, and abilities.

Communication Skills

Effectively communicate with patients and families regarding complex information related to their care.

Provide guidance and information to patients, caregivers, and their families as appropriate.

Collaborate effectively with medical, nursing, allied health professionals, and operational staff to ensure coordinated service delivery.

Maintain accurate record-keeping in accordance with local policies and NMC guidelines.

Promote and exemplify effective communication networks within the organization.

Clinical Governance and Service Enhancement

Collaborate with the Clinical Director to:

  • Provide clinical leadership to nursing team members as needed.
  • Investigate clinical incidents and complaints.
  • Develop and implement clinical audits to identify best practices and areas for improvement.
  • Support and supervise others to enhance their knowledge, skills, and abilities.
  • Contribute to quality improvement initiatives by identifying and initiating changes that lead to better evidence-based practices and improved care standards.

Adhere to ELM policies, procedures, and guidelines.

Educational and Personal Development

Work alongside the Operations Manager and Clinical Director to:

  • Create and maintain an outstanding learning environment.
  • Develop and implement various learning activities for the nursing team.
  • Support the professional growth of other nursing team members.
  • Comply with professional registration requirements.
  • Engage in personal development planning to enhance clinical skills and ensure ongoing professional growth.
  • Complete all statutory and mandatory training.
  • Participate in performance appraisals and regular reviews.
  • Ensure completion of all mandatory training in accordance with organizational policy.
