Position Overview

In response to our expanding operations, we are excited to announce openings for Senior Clinical Nurse Practitioners.

The core responsibility of this position is to function as an independent advanced practitioner, delivering exceptional and prompt patient-focused care.

The individual in this role will manage the assessment, diagnosis, treatment, referral, and discharge processes for a varied patient demographic presenting with minor injuries and illnesses, all while adhering to their professional competencies and established protocols.

This role aligns with that of a conventional in-hours Senior Clinical Nurse Practitioner in General Practice, distinguishing itself from the Out of Hours roles.

Key Responsibilities

  • Serve as an advanced practitioner, showcasing superior clinical skills and knowledge that surpass traditional nursing roles.
  • Conduct pre-scheduled nursing consultations similar to those in a standard GP practice.
  • Perform patient triage.
  • Engage in consultations, both in-person and via telephone, with patients across all age groups (from neonates to the elderly), utilizing independent clinical judgment to diagnose, treat, refer, and/or discharge patients.
  • Facilitate direct referrals to primary, secondary, and social services in accordance with our established pathways and protocols.
  • Administer wound care and dressings.
  • Manage chronic disease care for pre-scheduled patients when assigned to specifically qualified Nurse Practitioners.
  • Ensure the delivery of high-quality clinical care in a timely manner.
  • Prescribe, supply, and administer medications as necessary to meet patient needs.
  • Maintain thorough and accurate documentation.
  • Collaborate as an independent practitioner and as part of multidisciplinary teams to ensure comprehensive patient care.

Communication Skills

Effectively communicate complex care information to patients and their families.

Provide guidance and information to patients, caregivers, and families as appropriate.

Collaborate with medical, nursing, allied health professionals, and operational staff to ensure a coordinated service delivery.

Maintain accurate records in accordance with local policies and NMC guidelines.

Foster effective communication networks within the organization.

Clinical Governance and Improvement

Collaborate with the Clinical Director to:

  • Provide clinical leadership to nursing team members as needed.
  • Investigate clinical incidents and patient complaints.
  • Develop and implement clinical audits to identify best practices and areas for improvement.
  • Support and supervise colleagues to enhance their skills and knowledge.
  • Contribute to service quality improvements through the identification and initiation of evidence-based practice changes.

Adhere to ELM policies, procedures, and guidelines.

Educational Development

Work alongside the Operations Manager and Clinical Director to:

  • Create and sustain an exceptional learning environment.
  • Design and implement various educational activities for the nursing team.
  • Support the professional growth of nursing team members.
  • Comply with professional registration requirements.
  • Engage in personal development planning to enhance clinical skills and ensure ongoing professional growth.
  • Complete all mandatory training as per organizational policy.
  • Participate in performance appraisals and regular reviews.
