Job Overview

This position involves providing essential administrative assistance to the Fetal Cardiology department, with responsibilities spanning multiple sites.

Key Responsibilities

The following duties are indicative of the Patient Pathway Coordinator role and its significance within the service; they are not exhaustive, and the individual may be expected to perform additional or alternative administrative and managerial tasks to ensure the efficient operation of the department.

Responsibilities Include:

  1. Monitoring and assessing patient care and treatment progress in accordance with Trust and local policies.
  2. Effectively managing patient pathways and liaising with clinicians as necessary.
  3. Ensuring precise data entry and management.
  4. Identifying subsequent steps in the patient care pathway based on clinical requirements.
  5. Processing patient referrals in alignment with established timelines and policies.
  6. Coordinating administrative support for relevant teams to guarantee availability of necessary paperwork and clinical information.
  7. Responding to inquiries regarding patient care and treatment pathways promptly.
  8. Developing familiarity with the milestones associated with the assigned patient pathways.
  9. Participating in relevant meetings and executing actions related to patient care.
  10. Engaging in national audits and data collection to support quality improvement initiatives.
  11. Proposing enhancements to the patient care pathway and tracking processes.
  12. Monitoring patient progress and proactively addressing issues to expedite treatment.
  13. Resolving daily operational issues and escalating significant concerns to management.
  14. Ensuring the efficient transfer of patients and information between trusts and organizations, adhering to established communication protocols.
  15. Collaborating within a multi-disciplinary team to advance departmental objectives.
  16. Maintaining the security and proper tracking of health records in accordance with policy.
  17. Building effective working relationships with a diverse range of clinical and managerial personnel.
  18. Responsible for collecting and updating patient information in relevant databases to facilitate efficient pathway progress.
  19. Undertaking administrative tasks as required to support high-quality patient care.
  20. Actively participating in departmental development and meetings.
  21. Performing any other duties as necessary to ensure the smooth operation of the department.
  22. Completing required training to fulfill job responsibilities.

Person Specification

Qualifications/ Education

Essential

  • GCSEs or equivalent, including Mathematics and English.

Desirable

  • Qualification or training in customer service.

Experience

Essential

  • Demonstrated administrative experience.

Desirable

  • Experience in an administrative capacity within the NHS.
  • Experience in supervising staff.

Skills/Knowledge/Ability

Essential

  • Proficient IT skills, particularly in Microsoft Office applications such as Word, Outlook, and Excel.
  • Strong organizational and time management skills to meet deadlines.
  • Ability to manage challenging service users and situations effectively.
  • Excellent interpersonal skills for effective communication with a diverse range of multi-disciplinary staff.
