Job Summary

We are seeking a highly organized and detail-oriented Clinical Pathway Coordinator to join our team at Royal Free London NHS Foundation Trust. As a Clinical Pathway Coordinator, you will play a critical role in providing administrative support to our Community Diagnostic Centre, ensuring the smooth operation of our diagnostic services.

Key Responsibilities

  • Manage referrals received from referring trusts and ensure they are placed on an appropriate pathway.
  • Ensure effective communication channels are in place and that key stakeholders, including patients, families, and carers, are informed of communication details and hours of work.
  • Identify and resolve bottlenecks or delays in patients' pathways, escalating issues as necessary to the Admin Team Leader.
  • Coordinate and book appointments for patients in line with referring trusts' access policy.
  • Liaise with referring trusts by sending End of Day reports for each diagnostic modality.
  • Ensure patients understand their care pathway and are aware of appointments and any changes.
  • Monitor capacity across all modalities and escalate any capacity issues.
  • Ensure all appointments are compliant with the Trust Access Policy and national targets.
