About the Role

We are seeking a highly organized and detail-oriented individual to join our team as a Clinical Pathway Administrator in Nephrology and Renal. The successful candidate will be responsible for coordinating administrative processes, ensuring smooth patient pathways, and maintaining accurate records.

Key Responsibilities
  • Coordinate all administrative processes for patient pathways, ensuring timely and efficient management.
  • Ensure patient demographics and contact details are up-to-date on Cerner.
  • Access patient records using computerized tracking systems, maintaining confidentiality and adhering to data protection policies.
  • Provide clear instructions, reminders, and information to patients via phone calls.
  • Attend and participate in relevant meetings, contributing to effective communication and collaboration.
  • Provide patient navigation services for satellite and off-site locations, ensuring seamless care delivery.
  • Regularly check with Admissions to ensure patients are added to the elective waiting list.
  • Provide cross-cover for other patient navigators, maintaining a consistent and effective administration service.
  • Process confidential patient-sensitive documentation, handling concerns and complaints promptly and professionally.
Requirements
  • Demonstrable ability to meet the Trust Values.
  • Knowledge of medical terminology, English and Maths at GCSE level or equivalent, and secretarial experience.
  • Desirable: Diploma level qualification or equivalent experience, courses or further study attended to demonstrate evidence of personal development.
  • Previous experience working with the public and/or patients in a healthcare environment, interacting with all levels of staff, and managing and prioritizing own workload.
  • Excellent written and verbal communication skills, organizational skills, customer care practice, and understanding of patient confidentiality.
