Job Overview

Victoria Park Health Centre is seeking a dedicated and knowledgeable Clinical Pharmacist to join our team. In this role, you will operate under the daily supervision of the Lead Pharmacist, collaborating closely with GP Practice mentors and clinical leaders. Your expertise will be essential in enhancing medication management, supporting long-term health conditions, and facilitating effective communication across various healthcare settings to ensure improved patient care and outcomes.

This position is vital for maintaining high-quality, accessible healthcare services for our patients while providing pharmaceutical guidance to our healthcare team.

Key Responsibilities

  • Work collaboratively with the Lead Pharmacist and GP practice mentors to optimize medication use and enhance patient care.
  • Conduct comprehensive reviews of medication requests and repeat prescriptions, ensuring safe and effective medication management.
  • Provide expert pharmaceutical advice to prescribers and healthcare practitioners within the practice.
  • Engage in patient consultations, managing a caseload focused on optimizing medication use for patients with stable clinical conditions.
  • Deliver clinics and consultations, prioritizing conditions identified by the practice and working alongside other healthcare professionals.
  • Utilize technology and digital tools to empower patients in managing their health and medication.
  • Analyze data from practice systems to enhance population health and resource management.
  • Support GPs and practice nurses with medication-related queries and the implementation of new care pathways.
  • Collaborate with community pharmacists to improve medication management during transitions of care.

Qualifications and Experience

Essential:

  • Master's degree in Pharmacy and current registration with the GPhC.
  • Demonstrated commitment to ongoing professional development in clinical services.
  • Strong foundational knowledge of general practice and evidence-based medicine.
  • Independent Prescriber status.

Desirable:

  • Membership in the Royal Pharmaceutical Society or willingness to join.
  • Experience in managing patient consultations and working across multidisciplinary teams.
  • Understanding of primary care prescribing systems and funding pathways.
