About the Role

This is a fantastic opportunity for a pharmacist with hospital and/or primary care experience to join the University Hospitals of Leicester NHS Trust's Care Homes Pharmacy Team.

Key Responsibilities
  • Provide a medicines optimisation service to care home residents across the region and work across the interface to ensure the best outcome for these patients.
  • Deliver a clinical service to patients, including taking patient drug histories, rationalising prescribing, advising on and compiling treatment protocols, therapeutic drug monitoring, and monitoring compliance (in primary and secondary care).
  • Support the Clinical Management Group (CMG) Lead Pharmacist in maintaining and improving the pharmaceutical service to the CMG.
  • Be responsible for the delivery of an agreed standard of pharmaceutical care to patients to achieve the safe and cost-effective use of medicine.
Working Arrangements

This is a full-time role, but we are happy to consider other working patterns.
