Position Overview

This role presents an excellent opportunity for a pharmacist with experience in hospital and/or primary care settings to become a vital member of the LLR Care Homes Pharmacy Team.

This distinctive position entails collaboration across various healthcare settings, including hospital admissions wards, primary care environments, and care homes.

Our team is dedicated to providing specialized support for care homes, focusing on optimizing medication management for vulnerable populations, including those with multiple medications and high-risk residents. Responsibilities include conducting thorough medication reviews, performing medication audits, engaging in multidisciplinary team (MDT) collaborations, and supporting various care home initiatives throughout the region.

Successful candidates will have the chance to engage in innovative projects, contribute to research efforts, and influence the national discourse on pharmacy practices in care homes.

Key Responsibilities

  • Deliver a comprehensive medicines optimization service to residents in care homes across the LLR region, ensuring optimal patient outcomes.
  • Provide clinical services, including gathering patient medication histories, rationalizing prescriptions, advising on treatment protocols, monitoring therapeutic drug levels, and ensuring compliance in both primary and secondary care settings.
  • Assist the Clinical Management Group (CMG) Lead Pharmacist in enhancing the pharmaceutical services provided to the clinical management group.
  • Ensure the delivery of high standards of pharmaceutical care to achieve safe and cost-effective medication use.

This position offers flexible working arrangements, and we are open to discussing various working patterns.

Main Duties

  • Contribute to the provision of optimal pharmaceutical care to patients within the designated clinical directorate, care homes, and across healthcare interfaces.
  • Offer expert advice to patients and healthcare professionals regarding medication guidelines, particularly in complex cases and chronic disease management.
  • Participate in managing the relevant pharmaceutical budget through regular expenditure analysis and engage in ongoing drug usage audits to promote effective medication use.
  • Compile and present findings from interventions and audits as directed by senior pharmacists.
  • Provide training and educational support to healthcare staff within the organization and across the LLR region.
  • Actively engage in audit, research, and development initiatives within the department, sharing results through presentations and publications.
  • Prescribe and evaluate medications for therapeutic effectiveness, tailored to patient needs, adhering to evidence-based practices and established protocols.
  • Collaborate with other healthcare professionals in primary care, focusing on continuity of care and support for residents in care homes.
  • Identify patients in care homes at risk of hospital admission and implement effective medication reviews to mitigate this risk.
  • Perform additional duties as required, commensurate with the role.
