Pharmacy Technician - Focus on Patient Care

Key Responsibilities

The role involves delivering a ward-based medicines management service, which includes thorough and prompt medicines reconciliation, contributing to the safe and effective discharge of patients, and conducting audits. You will play a crucial role in ensuring that patients receive the correct medications at the appropriate times by collaborating with the broader multidisciplinary team and pharmacy services through effective communication and detailed handovers.

The successful candidate will be self-driven and organized, expected to contribute to service development and innovation. The position offers a significant degree of autonomy, requiring the ability to work independently as well as part of a team.

As a champion for pharmacy services and a steward of medication safety, applicants must possess excellent communication skills and the ability to build strong interpersonal relationships. You will also assist in the training and development of apprentices, assistants, pre-registration pharmacists, and technicians, and may be expected to undertake line management responsibilities.

This position will primarily be based at a designated healthcare facility; however, the successful candidate may occasionally be required to work at alternative locations.

The post holder will be expected to participate in core pharmacy services, including late shifts, weekends, bank holidays, and on-call duties as part of a rota.

Job Responsibilities

  • Coordinate medicines supply activities at the ward level, ensuring effective communication and collaboration with central dispensaries.
  • Oversee the supply of items for one-stop dispensing (once clinically verified by a pharmacist), which includes generating orders, dispensing, and placing medications in the patient's bedside cabinet while endorsing charts according to departmental policies.
  • Implement and maintain the Patients Own Drug (POD) scheme for the designated area, as well as self-administration protocols where applicable, in accordance with trust-wide policies.
  • Facilitate discharge planning and implementation in collaboration with the clinical pharmacist and ward staff, engaging with medical and nursing teams to identify patients for discharge, verifying medications against the TTO letter, and dispensing and relabeling as necessary.
  • Ensure accurate drug histories are obtained for identified patients through communication with patients/carers and/or by liaising with external organizations, including GP practices, community pharmacies, and nursing/residential homes.
  • Provide patient counseling for selected individuals and collaborate with the clinical pharmacist to ensure patients comprehend their discharge medications.
  • Work closely with ward staff regarding any relevant medicines management issues to ensure adherence to prescribed medications.
