We are thrilled to present an opportunity for a Medication Management Specialist to become a vital part of our healthcare team at Practice Plus Group, dedicated to providing exceptional pharmaceutical services.

In this position, you will have the chance to deliver top-tier healthcare to our patients, ensuring that efficiency and quality remain uncompromised.

Work Schedule

Monday to Friday

Three shifts from 07:30 to 18:00 and one shift from 07:30 to 15:30

Weekend hours

07:30 to 18:00

A commitment to work one weekend every four weeks is expected.

Part-time hours may be considered.

This role is full-time, offering an annual salary of up to £38,115 per annum FTE, based on experience.

Required Qualifications & Experience

  • NVQ Level 3 or equivalent qualification
  • Registration with the General Pharmaceutical Council
  • Current knowledge of Continual Professional Development (CPD)
  • Relevant post-qualification pharmacy experience

While not mandatory, experience in a hospital, community, or secure environment is preferred.

Responsibilities Overview

As a GPHC registered Medication Management Specialist with Practice Plus Group, you will:

Deliver exceptional patient-centered care, actively seeking and responding to patient feedback to ensure personalized care that meets their needs. Focus on providing a secure and efficient medicines management service, maintaining patient medical records to the highest standards and adhering to established operating procedures.

Provide non-judgmental care within professional standards, which may include timely medication administration and cost-effective prescribing as outlined in our health in justice medicines policy. Your role will be pivotal as a member of a multidisciplinary healthcare team.

Foster excellence by building strong relationships within our teams, partners, and stakeholders, ensuring a service that is trustworthy, safe, and commendable. Collaborate to make a positive impact on the lives of those we support, making Practice Plus Group a workplace where everyone feels valued and empowered to perform at their best.

Advocate for best practices, evidence-based care, and ongoing quality improvement, nurturing a culture of shared learning. Communicate complex or sensitive information with empathy and understanding to patients.

Share insights regarding pharmaceutical products, medicines management, drug protocols, and health education to ensure the multidisciplinary healthcare team is well-informed.
