Job Summary

We are seeking a skilled Care Coordinator to play a key role in providing support and ensuring residents receive the care they need. As a Care Coordinator, you will be responsible for co-ordinating care for residents of care homes, ensuring their needs are addressed.

Main Responsibilities

  • Identify and manage a caseload of patients in need of care packages.
  • Engage with patients, carers, and advocates to develop holistic care plans.
  • Provide guidance on managing health and accessing support services.
  • Liaise with local health and care services to co-ordinate care.
  • Maintain up-to-date records and ensure all care plans are followed.

Person Specification

Essential

  • Good general standard of education.
  • Experience working with people to empower them and improve their wellbeing.
  • Strong communication and interpersonal skills.
  • Ability to work on own initiative and manage time effectively.
  • Knowledge of healthcare systems, safeguarding, and confidentiality.

Desirable

  • Health and social care qualification.
  • Experience in a multidisciplinary team.
  • Familiarity with issues related to dementia and complex health needs.
  • Use of IT systems including EMIS and Microsoft Office.
