Position Overview:

We are seeking a dedicated and experienced Community Nursing Team Leader to enhance our leadership capabilities within the Brent community nursing framework.

Key Responsibilities:

  • Provide clinical oversight and direction in delivering exceptional care to patients in the community.
  • Manage the daily operations of the District Nursing team, ensuring adherence to high standards within the designated area.
  • Lead initiatives aimed at monitoring and elevating care standards while contributing to the overall growth of the adult nursing service.
  • Offer managerial and clinical guidance to community nursing and allied health professionals, serving as a role model for multidisciplinary team members.
  • Dedicate 50% of your time to clinical responsibilities as an expert practitioner, delivering specialized District nursing care while maintaining clinical competencies and achieving quality outcomes.
  • Ensure patient safety and clinical effectiveness by providing expert, evidence-based clinical advice and support to all staff in the locality.
  • Coordinate and oversee the provision of high-quality nursing care that aligns with identified patient needs and supports the healthcare team's objectives, all while operating within resource constraints.
  • Assist the Integrated Care service manager in delivering a cost-effective service.
  • Create and sustain a conducive learning environment to meet the educational needs of all staff.

Additional Responsibilities:

  • Manage patient care for individuals referred by local General Practitioners (GPs) to prevent unnecessary hospital admissions, ensuring comprehensive assessments, individualized care plans, and coordination of care with other involved agencies.
  • Maintain communication with GPs, participating in regular meetings to keep them informed of patient progress and collaborating on care planning to minimize the risk of inappropriate hospital admissions.
  • Demonstrate and uphold clinical credibility by providing high standards of clinical care based on current evidence-based practices, continuously reviewing and improving practices to ensure efficient care delivery.
  • Collaborate with the Clinical Development lead to design and implement training programs for District Nurses.
