Community Health Nurse

We are seeking a skilled and dedicated nurse to join our community nursing team at Central London Community Health Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care in the community, working closely with patients, families, and healthcare professionals to promote health, prevent illness, and manage chronic conditions.
Main Responsibilities- Provide skilled nursing care to patients in their homes, focusing on chronic disease management, end-of-life care, continence promotion, and wound management.
- Act as a Named Nurse, working closely with the District Nurse Team Lead to ensure continuity of care and effective communication with patients, families, and healthcare professionals.
- Conduct comprehensive assessments of patients' physical, psychological, and social care needs, using the Single Assessment Process to inform care planning and implementation.
- Develop and implement individualized care plans that promote self-care, prevent unnecessary hospital admissions, and support patients' independence.
- Registered Nurse (RN) with current NMC registration.
- Experience in community nursing or a related field, with a strong understanding of chronic disease management and community care principles.
- Excellent communication, organizational, and time management skills, with the ability to work flexibly and prioritize tasks effectively.
