We are seeking a skilled and dedicated Community Health Nurse to join our team at Central London Community Health Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care and contributing to the development of our community nursing service.
Main ResponsibilitiesAs a Community Health Nurse, your key responsibilities will include:
- Providing skilled nursing care to patients in their homes, based on principles of best practice and clinical evidence.
- Managing chronic diseases, end-of-life care, continence promotion, and wound management.
- Administering medication via syringe drivers and intravenous therapy, PEG management, urinary catheterization, and tracheostomy suctioning.
- Acting as a Named Nurse with the support and supervision of the DN Team Lead.
- Undertaking comprehensive assessments of patients' physical, psychological, and social care needs.
- Developing individualized care plans that promote self-care and prevent unnecessary hospital admissions.
To be successful in this role, you will need:
- A Registered Nurse (RN) qualification and NMC registration.
- Experience in community nursing or a related field.
- Excellent communication and organizational skills.
- A commitment to delivering high-quality patient care and contributing to the development of our community nursing service.
