Community Health Nurse

We are seeking a skilled and compassionate Community Health Nurse to join our team at Central London Community Health Trust. As a Community Health Nurse, you will provide high-quality nursing care to patients in their homes, promoting independence and self-care. You will work closely with the Community Nurse Team Lead and Deputy Team Lead to ensure seamless care delivery and coordinate patient care needs.
Main Responsibilities- Provide skilled nursing care to patients in their homes, including assessments, care planning, and implementation of care plans.
- Work collaboratively with the multidisciplinary team to ensure comprehensive care delivery.
- Identify and record new problems and report back to the DN Team Lead as appropriate.
- Act as a patient advocate and facilitate patient choices regarding nursing care.
- Develop and utilize specialist nursing skills, such as leg ulcer assessment and management, syringe driver set up, and intravenous drug administration.

Person Specification- Registered Nurse (RN) with NMC Registration.
- Experience of working with people with complex health needs and providing holistic care.
- Knowledge of PREP, infection control, and promotion of continence.
- Ability to work in a team, prioritize tasks, and delegate effectively.
