Job Summary

We are seeking a skilled and dedicated nurse to join our community nursing team at Central London Community Health Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care in the community, working closely with patients, families, and other healthcare professionals to promote health, prevent illness, and manage chronic conditions.

Main Responsibilities

As a Community Health Nurse, your key responsibilities will include:

  • Providing skilled nursing care to patients in their homes, community settings, and other locations as required
  • Conducting comprehensive assessments of patients' physical, psychological, and social needs
  • Developing and implementing individualized care plans to promote health, prevent illness, and manage chronic conditions
  • Working collaboratively with other healthcare professionals to ensure seamless care transitions and coordination
  • Providing education and support to patients and their families to promote self-care and independence
Requirements

To be successful in this role, you will need:

  • Current registration with the Nursing and Midwifery Council (NMC)
  • Experience in community nursing or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work in a variety of settings and adapt to changing priorities
