Key Responsibilities- Provide skilled nursing care to patients with complex health needs, working under the supervision of the Community Nurse Team Lead and Deputy Team Lead.
- Conduct comprehensive assessments of patients' physical, psychological, and social care needs, using the Single Assessment Process.
- Develop and implement individualized care plans that promote self-care, prevent unnecessary hospital admissions, and ensure continuity of care.
- Act as a patient advocate, facilitating their choices and promoting independence and self-care.
- Collaborate with the multidisciplinary team to provide holistic care that meets the needs of patients and their carers.
- Registered Nurse (RN) with NMC Registration.
- Experience of working with people with complex health needs, undertaking general assessments, and providing holistic care.
- Knowledge of PREP, infection control, and promotion of continence.
- Ability to work in a team, prioritize tasks, and delegate effectively.
- Car Driver.
