Community Nurse

We are seeking a highly skilled and compassionate Community Nurse to join our team at Hounslow and Richmond Community Healthcare NHS Trust. As a Community Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals in their own homes.
Key Responsibilities- Deliver high-quality nursing care to housebound patients with complex needs, enabling them to remain in their own homes.
- Work closely with specialist nurses, allied health, and social care professionals to meet the needs of patients without unnecessary attendance in A&E or hospital admission.
- Conduct first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care.
- Collaborate with the Community Nursing Team Manager to ensure seamless care to our patient group.

Person Specification- Level 1 Registered Nurse with valid registration with the NMC.
- Recent experience of Nursing at Level 5, or evidence of a back to nursing programme.
- Ability to converse articulately on current health issues, demonstrate knowledge in: Essence of Care, Team Working, professional standards including confidentiality and clinical governance, Adult Safeguarding, evidence of Continuing Professional Development, evidence-based clinical knowledge, health and safety and personal safety issues.
