Community Nurse

We are seeking a skilled Community Nurse to join our team at Shropshire Community Health NHS Trust. As a Community Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes or chosen place of residence.
Main Responsibilities- Conduct holistic assessments of patients' care needs and develop individualized care plans.
- Provide nursing care to patients with complex and simple care needs, including delegation of care to other team members as appropriate.
- Work collaboratively with other healthcare professionals to promote independence and timely discharge for patients.
Shropshire Community Health NHS Trust provides community-based health services for adults and children in Shropshire, Telford and Wrekin, and surrounding areas. We are committed to delivering excellent patient care and modern ways of working.
Person Specification- RGN NMC Registered
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Post registration general experience
- Experience of working in a multi-disciplinary/multi-agency health/social care environment
