Community Nurse

We are seeking a skilled and dedicated Community Staff Nurse to join our team at Shropshire Community Health NHS Trust. As a Community Staff Nurse, you will play a vital role in delivering high-quality patient care in the community, working closely with other healthcare professionals to promote independence and well-being.
Key Responsibilities- Conduct holistic assessments of patients' care needs, developing and implementing individualized care plans.
- Provide nursing care to patients in their own homes or chosen place of residence, promoting timely discharge and enabling people to realize their potential.
- Work collaboratively with other members of the multi-disciplinary team to promote independence and well-being.
- Develop and maintain effective relationships with patients, carers, and other healthcare professionals.
- RGN NMC Registered.
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes.
- Post registration general experience.
- Experience of working in a multi-disciplinary / multi-agency health/social care environment.
- Experience and knowledge of basic administration systems including Microsoft Office.
- Experience of successful partnership working arrangements with other services.
- Previous community experience.
