Job Title: Community Nurse Specialist

We are seeking a highly skilled and dedicated Community Nurse Specialist to join our team at Sussex Community NHS Foundation Trust. As a Community Nurse Specialist, you will be responsible for delivering high-quality care to patients in their own homes, working towards rehabilitation, self-management, and independence throughout the continuum of care.

You will participate in a team of colleagues supporting evidence-based care, facilitating staff learning and development. Your main duties will include diabetic management, wound management, palliative care, bladder and bowel care, and other continence needs.

We are looking for a candidate with a strong background in community nursing, excellent communication and negotiation skills, and the ability to demonstrate competency in a range of clinical skills. You will also have the ability to apply research-based practice and knowledge of current local policies informing community health and social care services.

Key Responsibilities:

  • Demonstrates competency in a range of clinical skills.
  • Is able to demonstrate the application of research-based practice.
  • Knowledge of current local policies informing community health and social care services.
