Job Title: Community Health Nurse Specialist

At Central London Community Healthcare NHS Trust, we are seeking a skilled Community Health Nurse Specialist to join our team. As a Community Health Nurse Specialist, you will provide high-quality nursing care to patients in various community settings, working under the supervision of the Community Nurse Team Lead and Deputy Team Lead.

The successful candidate will undertake community nursing duties, delivering nursing services and holistic assessments of patients' health needs using a person-centred approach. This includes providing quality and seamless nursing care to housebound patients, coordinating their care needs, and facilitating self-care for patients with long-term conditions.

Main Responsibilities:

  • To provide skilled nursing care based on principles of best practice and clinical evidence, including chronic disease management, end of life care, continence promotion, and wound management.
  • To act as a Named Nurse, taking continued responsibility for the holistic health needs assessment, care planning, evaluation, and implementation of healthcare, ensuring continuity of care following referral from acute care, rehabilitation units, GPs, and other agencies.
  • To undertake comprehensive assessments of patients' physical, psychological, and social care needs, providing individualised and person-centred care plans that promote self-care and prevent unnecessary hospital admissions.
  • To act as a patient advocate, facilitating their choices regarding nursing care and promoting independence and self-care as appropriate.

Requirements:

  • Registered Nurse (RN) with NMC Registration.
  • Recent clinical nursing experience negotiating and liaising within a multidisciplinary team.
  • Experience of working with people with complex health needs, supporting students and other learners, and undertaking general assessments using evidence-based care.
  • Previous experience in the Nursing process, care planning, and fluent record keeping.

Skills and Knowledge:

  • Knowledge of out-of-hospital care and the role of the Community Nurse.
  • Knowledge of risks involved in working in the community.
  • Knowledge of infection control and general wound care.

Other:

  • Ability to organise and participate in weekly work allocation, prioritise, and delegate effectively.
  • Ability to assess situations and act promptly, communicate well verbally and in writing, and be a car driver.
