Urgent Community Nursing Opportunities in West London

Community Services Institute Inc is seeking experienced Community Nurses to join our team in West London. We are working in partnership with the NHS trust to provide high-quality healthcare services to the community.

Job Summary

We are looking for skilled Community Nurses who can work independently and as part of a team to provide patient-centered care. The ideal candidate will have a strong understanding of community health and be able to work in a fast-paced environment.

Key Responsibilities
  • Provide high-quality patient care in a community setting
  • Work collaboratively with other healthcare professionals to achieve patient outcomes
  • Develop and implement care plans that meet the needs of patients
  • Conduct health assessments and provide health education to patients
  • Participate in quality improvement initiatives to enhance patient care
Requirements
  • NMC Registered
  • Car owner
  • Enhanced DBS within 1 year
  • Experience of evidence-based care
  • Mandatory Training
