Job Summary

We are seeking a highly skilled and experienced Nurse to join our team as a Senior Community Nurse Leader. This is an exciting opportunity for a motivated and dynamic individual to work closely with our clinical teams to deliver high-quality care and improve patient outcomes.

Main Responsibilities
  • Lead and manage a team of community nurses to provide high-quality patient care and support.
  • Develop and implement quality improvement initiatives to enhance patient outcomes and experience.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
  • Provide clinical leadership and guidance to junior staff members.
  • Contribute to the development and implementation of community care pathways and services.
Requirements
  • Registered General Nurse (RGN) or equivalent qualification.
  • Post-registration experience in community nursing, preferably in a leadership role.
  • Strong clinical skills and knowledge of community care services.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Basic computer skills and proficiency in using electronic patient records.
