Job Title: Band 6 Senior Community Nurse

We are seeking a highly skilled and experienced Band 6 Senior Community Nurse to join our team at North East London NHS Foundation Trust.

About the Role

The successful candidate will be responsible for leading and managing a Community Nursing Team, working closely with GPs, other professionals, and statutory and voluntary agencies to deliver high-quality care to patients and their families in the community.

Main Duties
  • Lead and manage a Community Nursing Team, ensuring the provision of a high standard of nursing care to patients and their families.
  • Work as part of the integrated primary/community health care team, working within and across professional and organisational boundaries.
  • Liaise with GPs, other professionals, and statutory and voluntary agencies to deliver individualised care to patients.
  • Provide leadership to the community nursing team, working without direct supervision.
Person Specification

We are looking for a candidate with:

  • District nurse qualification, certificate, DiP, or Degree.
  • Current NMC Registration as a Registered Nurse (RN).
  • Nurse/Prescriber/Non-Medical Prescriber qualification.
  • Evidence of professional updating.
Desirable Qualifications and Experience

We would welcome applications from candidates with:

  • Relevant post-registration course and experience, e.g. Palliative Care, Diabetes, and Tissue Viability.
  • Mentor Qualification.
  • 3 years post-registration experience.
Knowledge and Skills

The ideal candidate will have:

  • An awareness of NHS Plan, NSF, and clinical governance priorities.
  • Awareness of the content and application in practice of the NMC Code of Professional Conduct.
  • A good range of clinical skills, including assessment.
  • Understand the importance of chronic disease management and rehabilitation in the community.
