Job Summary

We are seeking a highly skilled and compassionate Care Coordinator to join our team at PeoplePlus. As a Care Coordinator, you will play a vital role in providing high-quality information, advice, and support to individuals who are considering participating in Self-Directed Support Schemes, Independent Living Schemes, Personal Health Budgets, and self-funded/private clients.

Key Responsibilities

  • Provide a person-centred service that enables individuals to focus on the outcomes they wish to achieve, supporting with establishing a care and/or support plan that helps the person meet these outcomes in a cost-effective and safe manner.
  • Manage a caseload within a geographic area, including visits to individuals' homes, local authority offices, and other community locations.
  • Assist individuals to manage their obligations and responsibilities in relation to their chosen care package.
  • Obtain and collate information for caseload management and reports on a daily basis, utilizing chosen databases.
  • Maintain accurate records of all communication with individuals utilizing chosen databases.
  • Ensure all information given is clear and concise and in a format that meets the individual communication needs of the person being supported.
  • Ensure all information is factually correct and in accordance with PeoplePlus guidelines and current legislation.
  • Organize and attend meetings and/or reviews with individuals, colleagues, and other professionals as required.

Requirements

To be successful in this role, you will need to have a background in social care and experience working with vulnerable individuals. You will also need to be able to work remotely and have excellent communication and organizational skills.
