About the Role
We are seeking a highly skilled and compassionate Care Coordinator to join our team at PeoplePlus. As a Care Coordinator, you will play a vital role in providing high-quality information, advice, and support to individuals who are considering participating in Self-Directed Support Schemes, Independent Living Schemes, and Personal Health Budgets.
Key Responsibilities
- Provide person-centred services that enable individuals to focus on the outcomes they wish to achieve, supporting them in establishing a care and/or support plan that helps them meet these outcomes in a cost-effective and safe manner.
- Manage a caseload within a designated geographic area, including visits to individuals' homes, local authority offices, and other community locations.
- Assist individuals in managing their obligations and responsibilities in relation to their chosen care package.
- Obtain and collate information for caseload management and reports on a daily basis, utilizing designated databases.
- Maintain accurate records of all communication with individuals, utilizing designated databases.
- Ensure all information given is clear and concise and in a format that meets the individual communication needs of the person being supported.
- Ensure all information is factually correct and in accordance with PeoplePlus guidelines and current legislation.
- Organize and attend meetings and/or reviews with individuals, colleagues, and other professionals as required.
Requirements
- Background in social care with experience in supporting individuals with disabilities.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
- Proficiency in utilizing designated databases and software.
