Healthcare Coordination Specialist

We are seeking a highly organised and proactive individual to join our dynamic Urgent Community Response (UCR) Team as a Coordination Support Administrator. This vital role is key to the smooth and efficient delivery of urgent community care, enabling patients to receive timely interventions that prevent hospital admissions and support recovery at home.

Main Responsibilities
  • Act as a key point of contact for the UCR service, managing urgent referrals, coordinating communication between clinicians, patients, and external services.
  • Provide comprehensive administrative support, including diary management, scheduling patient visits, and maintaining accurate records of patient care.
  • Prioritise and coordinate rapid response team activity, ensuring urgent cases are actioned swiftly and efficiently.
  • Liaise with GPs, community services, and other healthcare professionals to ensure smooth communication and continuity of care.
  • Assist in the collection and reporting of data for service audits, performance monitoring, and quality improvement.

Requirements
  • Education to GCSE or equivalent with Grade C or above in English and Maths
  • NVQ3 in administration/RSA3 secretarial or equivalent experience
  • Diploma level Business/Administration/IT qualification or proven equivalent experience
  • Advanced IT qualification
Skills
  • Able to communicate professionally and effectively
  • Ability to work calmly and methodically under pressure
  • Ability to identify, coordinate, and organise health and social care support within required timescales
  • Knowledge of NHS procedures in particular those related to urgent response services
