POSITION SUMMARY

As a Community Healthcare Nurse, you will deliver care to patients receiving tube feeding in their residences. You will cultivate strong partnerships with caregivers and healthcare professionals, including dietitians, general practitioners, and local nursing staff.

This position is field-based and primarily serves Adult Patients in Hampshire and its neighboring regions. To qualify for this role, you must be a Registered Nurse (RGN, RSCN, or RNLD) with a minimum of 12 months of experience and reside in Hampshire or nearby areas. A full UK Driving License is also required; a company vehicle, fuel card, and a lunch allowance will be provided while you are in the field.

Key Responsibilities :

Develop and implement comprehensive training programs for patients, caregivers, and healthcare professionals to facilitate safe hospital discharges and ensure ongoing quality care at home. Conduct regular patient assessments and be available to assist with inquiries, concerns, and troubleshooting needs.

Following our recent success in securing the NHS contract, along with expanding our services in the Hampshire area, this role is crucial for maintaining our high standards of patient care.

Candidate Profile

QUALIFICATIONS

We are looking for candidates who can demonstrate:

The ability to work autonomously while effectively managing your schedule. A solid understanding of the UK Healthcare landscape, particularly the NHS. Proficiency in communication with both internal and external stakeholders, including patients and healthcare professionals.

Familiarity with enteral tube feeding is advantageous, but we will offer thorough training in this area.
