About the Role

As a Nutricia Community Nurse, you'll play a vital role in providing compassionate care to patients who require tube feeding at home. You'll build strong relationships with caregivers, healthcare professionals, including dietitians, general practitioners, and local nurses, ensuring seamless coordination of patient care.

This field-based position focuses on supporting adult patients in Hampshire and the surrounding areas.

Your Responsibilities

  • Develop and deliver comprehensive training programs to patients, caregivers, and healthcare professionals, facilitating safe hospital discharges and ongoing quality care at home.
  • Conduct regular patient reviews, providing support, addressing concerns, and resolving any issues that may arise.

About You

You'll thrive in this role if you possess a genuine passion for patient care and a commitment to delivering exceptional service. We seek individuals who are:

  • Registered Nurses (RGN, RSCN or RNLD) with at least 12 months of experience.
  • Residing in Hampshire and the surrounding areas.
  • Possess a full UK Driving License.
