About the Role

We are seeking a highly skilled and motivated Registered Nurse to join our Community Team in Trowbridge. As a Community Nurse, you will be responsible for delivering high-quality care to patients in their own homes and residential settings.

Main Responsibilities

  • Assess and care plan for patients with complex needs
  • Work collaboratively with a multidisciplinary team to provide comprehensive care
  • Develop and maintain relationships with patients, families, and carers
  • Contribute to the development of community-based services and initiatives

Requirements

  • Registered Nurse qualification and current registration with the NMC
  • Experience of working in a community setting and with patients with complex needs
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
