About the Role

We are seeking a highly skilled and motivated Registered Nurse to join our Community Team in Trowbridge. As a Community Nurse, you will play a vital role in delivering high-quality care to patients in their own homes and residential settings.

Main Responsibilities

The successful candidate will be responsible for assessing and planning care for patients, working closely with a multi-disciplinary team to provide comprehensive care. You will also be expected to develop and maintain strong relationships with patients, families, and carers, as well as contribute to the development of the team and service.

Requirements

  • Registered Nurse qualification
  • Experience of working in a community setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work a shift pattern, including weekends
