Position Overview

The Clinical Nurse Specialist will play a pivotal role in the execution of strategies aimed at the proficient management of patients with chronic conditions, immunization programs, and screening initiatives.

This role encompasses various clinical domains, including health promotion, management of chronic illnesses, preventive health measures, and conducting well-woman and well-man clinics. Additionally, the nurse will collaborate with the management team to review and enhance clinical policies and procedures.

Key Responsibilities

The ideal candidate will be a registered nurse with a background in primary care. Responsibilities include:

  1. Ensuring the provision of safe and effective nursing care to the patient population.
  2. Collaborating with the entire practice team to deliver routine nursing services.
  3. Supporting practice partners and the clinical team in achieving clinical performance indicators.

This position is structured for 30 hours per week.

Core Duties

The following are the primary responsibilities of the Clinical Nurse Specialist:

  1. Design and implement health promotion and wellness initiatives.
  2. Evaluate and manage treatment plans for patients with chronic diseases.
  3. Identify and support patients at risk of developing long-term health issues.
  4. Deliver routine nursing care in accordance with clinical guidelines and standards.
  5. Provide wound care and travel medicine services.
  6. Request and process pathology services as necessary.
  7. Maintain accurate clinical records and ensure compliance with relevant legislation.
  8. Support the clinical team in emergency situations and safeguarding matters.

Qualifications

Essential:

  • Registered Nurse qualification.
  • Experience in a primary care setting.
  • Proficiency in chronic disease management.

Desirable:

  • Postgraduate qualifications in Primary Care.
