Main duties of the job
You will provide specialist support and advice to patients acting as their Keyworker. You will oversee and co-ordinate care and personalise the patient pathway to deliver a supportive nursing service for patients and their families undergoing in-patient and out-patient treatment at The Christie. You will be responsible for delivering specialist urology cancer care across organisational, professional, and geographical boundaries both autonomously and as part of a highly valued member of the urology surgical oncology team.

MAIN DUTIES AND RESPONSIBILITIES
CLINICAL
1. Perform a comprehensive Key Worker role for penile cancer patients during their care pathway at the Christie Hospital and provide expert specialist advice for patients, relative and carers in relation to a cancer diagnosis and to other health care professionals.
Develop and maintain advanced clinical skills in assessment, and knowledge of diagnosis and treatment options for the patient. Liaise with other Clinical Nurse Specialists, Cancer Pathway Coordinators, and other professionals, as well as those from referring units, to ensure effective transfer of information across different NHS organisations and smooth pathway of care. Collect, collate, evaluate and report information, maintaining accurate patient records in line with clinical governance and risk management. Work independently managing own case load of patients Use advanced communication skills in a variety of settings, including discussions around patient care and treatment plans with patients, carers, relatives and members of the multi-disciplinary team. Provide emotional support to patients, their families and staff during difficult situations breaking bad news or following an unexpected event Involve patients/carers in the planning and delivery of care and development of services to enhance the patient/carer experience. Develop and run nurse led clinics and undertake highly specialised technical and/or practical skills, including advanced health assessments and non-medical prescribing to ensure optimum patient management. Will be expected to deliver clinics both on Christie main site, in outreach clinics in other NHS organisations and in community based practices. Apply evidence based research to practice. Act always in accordance with the NMC code of conduct and guiding documents Be an active member of the appropriate site specific MDT, utilising excellent communication skills, enabling maintenance of a high quality service and ensuring timely management and resolution of unexpected events and situations.LEADERSHIP
1. Provide specialist, expert clinical knowledge to clinical colleagues, patients and carers/relatives both within and outside the Trust.
2. Play an integral role in the operational and strategic development of specialist services for both in and out patients

EDUCATION
Identify learning needs, plan, implement and evaluate programmes of education to meet identified need. To attend meetings and conferences as appropriate ensuring that you are fully conversant with current practices both within the Trust and within specialist urology oncology services locally and nationally. To publish and present relevant clinical practice, audit and service developments. Participate in the education of pre and post- registered health care professionals/medical staff. Ensure own compliance with regard to mandatory training, non-medical prescribing and revalidation requirements. Receive clinical supervision as part of own professional development. Maintain up to date skills, knowledge and awareness of professional issues. Participate in planned programmes of appraisal. RESEARCH & AUDIT1. Identify the need for and undertake research, clinical audit and benchmarking, in collaboration with the team in order to improve effectiveness of patient care.
2. Disseminate research and audit findings through presentation to professional groups and publication.
3. Contribute to the writing of protocols relevant to own clinical speciality
4. To promote and facilitate evidence based practice and clinical audit within the Trust, raising the profile and enhancing the contribution of urological nursing.
