Psychosis Care Coordinator for Early Intervention

Watford, Hertfordshire, United Kingdom Hertfordshire Partnership University NHS Foundation Trust Full time

Position Overview

We are seeking a qualified professional in social work, occupational therapy, or mental health nursing.

If you possess experience in social work, occupational therapy, or mental health nursing, we invite you to consider joining our dedicated team within the Early Intervention in Psychosis (EIP) Service - PATH (Psychosis: Prevention, Assessment, and Treatment in Hertfordshire). Our focus is on individuals encountering their first episode of psychosis, and we are dedicated to providing tailored and impactful care.

This role will be based in the western region of Hertfordshire, specifically in St Albans & Harpenden.

We encourage applications from candidates interested in full-time, part-time, or job-sharing arrangements. We are committed to promoting flexible working options for our staff and welcome discussions regarding flexible working patterns to help maintain a healthy work-life balance.

This position offers a unique opportunity to enhance your leadership and delegation skills while benefiting from close collaboration with colleagues as part of a small team.

Additionally, you will receive training as a family intervention therapist.

Main Responsibilities

As a key member of a skilled multi-disciplinary team, you will:

  • Collaborate with colleagues to deliver our PATH service in the St Albans and Harpenden area.
  • Focus on a designated locality rather than the entire county.
  • Engage in community-based work, visiting clients in their homes and local hub facilities.
  • Undergo training as a family intervention therapist.

Your passion for delivering recovery-oriented and collaborative care planning will be essential, as you work creatively and flexibly with individuals to help them achieve their personal goals.

Our unique 'Caseload Mini Team' model fosters a supportive environment for collaborative efforts. You will take on the role of Leader of the Mini Team, partnering with a Band 4 & 5 colleague to manage the caseload effectively.

Incentives

If you are a qualified social worker, this position includes a Golden Hello of £3,500 for new employees, with £1,500 upon commencement and an additional £2,000 after completing one year of employment.

About Our Organization

We are one of only five mental health trusts to receive an overall rating of 'Outstanding' from the Care Quality Commission.

Our team of over 3,500 staff members provides health and social care for individuals with mental health challenges, physical health issues, and learning disabilities across Hertfordshire, Buckinghamshire, and Norfolk, delivering services within the community and various inpatient settings. Our approach is grounded in choice, independence, and equality, with our Trust values embedded throughout:

Welcoming. Kind. Positive. Respectful. Professional.

Learning and Development Opportunities

  • You will participate in a structured 12-week induction program, providing access to specific learning and development opportunities related to First Episode Psychosis (FEP).
  • Your professional growth is our priority, and we will offer various avenues for career advancement, including personal coaching and a robust supervision and appraisal program.
  • You will be encouraged to develop your leadership skills through tailored training and high-quality professional support.
  • As part of an innovative and evolving service, we are committed to continuous quality improvement, welcoming your ideas for service enhancement.
  • You will collaborate with a team possessing specialized knowledge and experience in FEP, with access to comprehensive internal training during your first year.
  • Delivering the FEP Pathway provides an opportunity to significantly impact service users' lives, equipping them with skills for self-management and fostering independence.
  • You will engage with individuals with diverse needs at various stages of their illness and recovery.
  • We offer a competitive benefits package, including access to salary sacrifice schemes, discounts, a lease car scheme, and excellent pension provisions with up to 42 days of leave per year.
