Psychosis Care Coordination Specialist

Watford, Hertfordshire, United Kingdom Hertfordshire Partnership University NHS Foundation Trust Full time

Position Overview

This role is ideal for a licensed social worker, occupational therapist, or mental health nurse.

If you possess experience in social work, occupational therapy, or mental health nursing, we invite you to become a vital part of our Early Intervention in Psychosis (EIP) Service at Hertfordshire Partnership University NHS Foundation Trust. Our focus is on providing tailored and impactful care to individuals encountering their first episode of psychosis.

This position will be situated in the West of Hertfordshire, specifically within a designated area.

We encourage applications for full-time, part-time, or job-sharing arrangements. Our commitment to flexible working practices allows us to support our staff in achieving a balanced work-life dynamic.

This role presents a unique opportunity to enhance your leadership and delegation capabilities while collaborating closely with team members in a supportive environment.

Additionally, you will receive training as a family intervention therapist.

Main Responsibilities

As a key member of a proficient multi-disciplinary team, your responsibilities will include:

  • Collaborating with colleagues to deliver our PATH service effectively.
  • Managing a specific locality rather than the entire county.
  • Engaging with the community, including visits to clients' homes and local hubs.
  • Receiving training as a family intervention therapist.

Your passion for recovery-oriented and collaborative care planning will be essential as you work creatively with individuals to help them achieve their personal objectives.

Our unique 'Caseload Mini Team' model fosters collaborative efforts, where you will lead a mini team alongside a Band 4 & 5 colleague to manage caseloads collectively.

Compensation

For qualified social workers, this position includes a Golden Hello of £3,500 for new employees, with £1,500 upon starting and an additional £2,000 after completing one year of service.

About Our Organization

We are one of only five mental health trusts to receive an overall rating of 'Outstanding' from the Care Quality Commission.

Our dedicated team of over 3,500 staff members provides comprehensive health and social care services for individuals facing mental health challenges, physical health issues, and learning disabilities across multiple regions. Our services are grounded in principles of choice, independence, and equality, reflecting our Trust values:

Welcoming. Kind. Positive. Respectful. Professional.

Learning and Development Opportunities

  • A structured 12-week induction program tailored to First Episode Psychosis (FEP) learning.
  • Access to personal coaching and professional development resources.
  • Encouragement to enhance leadership skills through specialized training.
  • Involvement in a creative and evolving service focused on continuous quality improvement.
  • Opportunities to work with a team possessing specialized knowledge in FEP.
  • Access to competitive employee benefits, including salary sacrifice schemes and an excellent pension plan.

We look forward to welcoming individuals who are committed to making a difference in the lives of those we serve.
