We are seeking a highly skilled and experienced Clinical Coordination Specialist to join our team at Rennie Grove Peace Hospice Care. As a key member of our small team of senior nurses, you will be responsible for the clinical coordination of care for patients and families referred to our hospice.
Key Responsibilities- Be the point of contact for patients, families, clients, and professionals who contact the hospice, dealing with all clinical enquiries.
- Develop the Coordination Centre's nursing role to ensure it provides a responsive, evidence-based, and high-quality service to beneficiaries and professionals.
- Work as a Specialist Palliative Care Clinical Nurse Specialist in a different Rennie Grove Peace clinical setting on a monthly basis to maintain and develop skills.
- Play an active part in promoting the Rennie Grove Peace service to others, including external Health and Social Care professionals and wider members of the public.
- Contribute to the wider strategic development of the hospice and help to translate local and national strategy relating to hospice care and enable Rennie Grove Peace to implement and deliver an effective service.
- Act as a Rennie Grove Peace Hospice Care ambassador, actively demonstrating our values and positively promoting the specialist care we provide.
- Degree Level or modules in Palliative Care or another relevant subject.
- Proven palliative care experience.
- IT literate, including a good knowledge of Excel, Word, and a clinical IT system.
- Advanced assessment skills and specialist symptom control knowledge to plan, implement, and evaluate nursing care.
- Full, current valid UK driving license.
