Job Summary:

Key Responsibilities:

  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.

Requirements:

  • Registered Nurse with a strong clinical background and experience in patient care coordination.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Strong problem-solving and critical thinking skills.
  • Proficiency in electronic medical records and other relevant software systems.
