To provide non-medical led clinical management for patients requiring care across a number of specialities.

The trainee advanced clinical practitioner (TACP) will work within a defined scope of practice and to agreed clinical standards applied within a Trust wide and local setting. The role will be multifactorial and include non-medical prescribing OR training in advanced management of pathophysiology and pharmacology, advanced clinical assessment of all systems, diagnostic skills and understanding of investigations. The role will improve the quality of, and access to health care provision and strengthen professional leadership.

The trainee advanced clinical practitioner will only provide clinical care for adult patients.

To practice as highly specialist clinician for patients with complex health conditions and/or at risk of imminent hospital admission. To offer holistic assessment and treatment for patients in their own home (whenever appropriate) to maximise health and wellbeing and minimise reliance on in-patient services and primary care physicians.

We support the daily function of the Urgent Community Response service, offering clinical support on triage of referrals and clinical assessments to our colleagues as required.

We manage a dynamic caseload of patients with complex chronic conditions for proactive case management. This work aims to build an ethos of empowerment for our patients and assist them in managing their own healthcare with individualised support.

We work closely as a team to ensure the multiple facets of our role can be undertaken with a supportive and flexible dynamic and to maintain highly specialist clinical knowledge and skills.
