Come and join our ever-expanding team of clinicians here at FCMS Urgent and Emergency Care Doncaster We currently have a range of Advanced Nurse Practitioners, Advanced Clinical Practitioners and Emergency Care Practitioners delivering high quality care to the people of Doncaster 24 hours a day, 7 days a week. The population of Doncaster now exceed 300,000 with a large variety of acute medical conditions our ECP Service offers autonomous clinicians who can assess, diagnose, and treat in the community. Where needed we have created strong links with our local trust to ensure referrals into hospital are streamline, working to avoid A&E admissions, if possible, by utilising the same day emergency care pathways. We pride ourselves on an open and honest culture and show dedication towards supporting our peers, working with staff to maximize potential, we value new ideas and welcome change. To benefit the people of Doncaster we want to introduce a Frailty ACP into the visiting team, all patients needing assessment in the community fit the definition of frail for many reasons and we believe this specialised level of knowledge and experience would benefit the team, patients, and the service we provide.

Role Purpose: Our Urgent & Emergency Care services in Doncaster continue to grow and FCMS are leading the way with the integration of our services with other hospital and community providers. FCMS operates the Urgent Treatment Centre based at Doncaster Royal Infirmary, Doncaster out of Hours GP service, ECP home visiting service and Same Day Health Centre in Doncaster Town Centre. The Same Day Health Centre provides care for all patients and is open 10am - 10pm 7 days a week, 365 days a year. Appointments are telephone booked following assessment by clinicians. FCMS is also part of the Doncaster Wound Care Alliance providing wound care to patients within the catchment area of Central Doncaster; twice weekly a wound care clinic is held for the most vulnerable in Doncaster by our fully trained specialist wound care nurse. There will be a requirement to work overnights, weekends and bank holidays as required.

RESPONSIBILITIES

Key areas of responsibility will include:
Clinical

The post holder will:
Assess diagnosis, plan and implement programmes of evidenced based care for the client group, undertaking re-evaluation and implementation of treatment plans as appropriate.

Ensure interventions are undertaken in line with best practice guidance.

Undertake advanced skills appropriate to the role, ensuring clinical competencies are maintained in accordance with framework for advanced clinical practice.

Provide advice, information, knowledge and support to staff.

Identify local, demographic, cultural and environmental factors that influence the health and quality of life.

Incorporate advanced practice in line with professional regulatory body.

To provide appropriate non-medical prescribing support for the organisation in adherence to Clinical Governance framework and in adherence to the agreed policy.

Undertake annual audit of prescribing practices.

Ensure that child protection and vulnerable adult legislation is adhered to within their own practice and the service.

Assist with the development and evaluation of policies/guidelines/protocols for advanced and specialist clinical practice

Continually review and evaluate own role and clinical practice
