The Acute Medical Unit provides emergency medical care for patients who are requiring admission under the expert care of our admitting emergency medical physicians within our trust. Medical Admissions is a 53 bedded unit that cares for a range of acutely ill patients requiring medical investigations and treatment in a fast-paced environment, with a turnover of 30-40 patients a day. We have a commitment to ensuring that every patients experience on admission is of the highest quality. We require a discharge progress co-ordinators who are energetic, forward thinking, flexible, self-motivated and who have care, compassion and courage at the core of their vision. We provide our patients with a true multi-disciplined team that can support our patients in their health needs with our own ward based therapy team and our goal is this will follow through to them being safely discharged to their home or next place of care.
To supervise and support members of the ward team to ensure that all patients receive the appropriate care in relation to their discharge off our ward.
To maintain a compassionate, caring and safe environment for all patients and their families/carers.
To act as an advocate for the patients and their families and or carers, providing them with information and support and directing them to the relevant support services where appropriate.
To plan effective discharge from admission, liaising with the multidisciplinary team, other departments and community services as necessary.
To use electronic patient systems i.e. PAS in accordance with Trust policy and procedure.
If you are looking for Professional and Personal development then we will support you to achieve this. There is a clear development structure in place, which includes senior ward level nursing as well as Clinical Nurse specialists involved in delivery of care within the emergency medical speciality. We actively encourage training and development with Mentorship and Care of the Critically Ill patient degree modules on offer.
Sustainability is integral to the Trust achieving the NHS Net Zero target. All staff are therefore actively encouraged and supported to implement new ways of working within their field of expertise.
You must be able to work as part of team and autonomously when required, and be committed to delivering a planned discharge of a high standard. We work closely with our colleagues in the Emergency Department and on The Same Day Medical Assessment Unit.
