Discharge Progress Co-ordinator

We are looking to recruit a new Discharge Progress Co-ordinator to work on the Trauma Unit. The ward is a 56 bedded Trauma and Orthopaedic Unit based at Royal Cornwall Hospital. We have a hard working MDT, and have a variety of different patients everyday, no day is the same.
We are looking for pro-active, highly motivated individuals who can work autonomously and as part of a large multidisciplinary team. The applicant will be expected to be enthusiastic, organised and have excellent communication and IT skills.
The role will focus on supporting flow of our trauma and orthopaedic patients to ensure all discharges are timely and meet the patients’ needs by liaising with a number of members of the multi
- disciplinary teams.
This is a full-time role for 37.5 hours per week
Co-ordinate all stages of the patient journey, from admission to discharge.
Proactively support and facilitate the work of the multi-disciplinary team (MDT) in delivering the best outcome for the patient; informing and communicating full outcomes of assessments and discharge plans within the clinical area relating to patient discharge/ flow.
Support the Service Manager/Clinical matron with implementing changes to improve patient experience and discharge experience.
The role is an advocate for promoting discharge and works closely with the multidisciplinary teams, including Trust colleagues, Clinical Site Management, Flow Hub, Social Care Providers, our Patients and their families, to proactively plan, prepare and support discharge activity.
Ensure electronic systems are updated in real time supporting the overall move away from paper records.
Will be a member of the Clinical Site Team but work to a specific ward or wards; occasionally may need to work in alternative areas and travel between sites to provide cross cover
