The Discharge Progress Co-Ordinator role is an advocate for promoting discharge and works closely with the multidisciplinary teams, including Trust colleagues, Clinical Site Management, Flow Hub, Social Care Providers, our Patients and their families, to proactively plan, prepare and support discharge activity.

In this role, you will be responsible for ensuring electronic systems are updated in real time supporting the overall move away from paper records. You will also be required to work as a member of the Clinical Site team but work to a specific ward or wards, occasionally may need to work in alternative areas and travel between sites to provide cross cover.
