Crescent Providence Surgery, our founding Practice, has been rated Outstanding by CQC.

It also enables us to employ a significant multi-disciplinary team including Pharmacists, Advanced Nurse Practitioners and Paramedics (for pre-assessment screening and home visits) all of whom reduce the clinical workload for GPs.

We have an exciting opportunity for a
Recalls and QoF Administrator to work across our surgeries. We are looking for someone to work
30-32hrs per week and will be based at our
Woodlea Surgery.


Your role will be primarily to assist the recall team in maintaining the recall system for chronic disease management and vaccination services in line with NHS England and CCG specification.


KEY RESPONSIBILITIES - ADMINISTRATION

  • Assist in maintaining recall system for chronic disease management.
  • Data input related to QoF requirements.
  • Using SystmOne reporting.
  • Maintain cervical screening protocol.
  • Provide general support to QoF Coordinators.
  • Provide organisational and administrative support as necessary.
  • Maintain and update documents in relation to position.
  • Receive, collate and review data from a variety of sources provided by locations throughout the month.
  • Develop effective relationships with internal colleagues and external stakeholders/customers.
  • Produce documents such as letters, reports, tables, monthly statistics and spreadsheets to a high standard of presentation and accuracy. Sometimes this will be from written draft, sometimes the jobholder will be required to produce from scratch.
  • Assist the surgery by covering reception when necessary.
  • Undertake and assist with adhoc duties as directed by the QoF Coordinators and Primary Care Contracts Manager.
  • Assist with preparation for CQC, CCG,NHS E, or internal quality inspections.
  • Assist with projects to improve the quality of services provided in surgeries.
  • Assist in organising of seasonal/adhoc campaigns (i.e. Health Promotion, QOF).
  • Conduct necessary research as needed, often via auditing sites, and clinical systems.
  • Carry out any other tasks which might be reasonably expected in support of the business.

KEY RESPONSIBILITIES - CLINICAL

  • Arranging appointments for patients who require annual reviews
  • Assessing home blood pressure readings and take appropriate action
  • Supporting our immunisation programmes and assisting with our seasonal campaigns including COVID and Flu. Ensuring there are available appointments for the patients and liaising with the rota administrator. Monitoring vaccination stock at each site to ensure there is enough available for patients and liaising with the stock administrator to acquire more if/when required.
  • Assisting the pharmacy team with their monitoring schedule for patients repeat medications. Including adding the relevant blood tests to the patients record using ICE and then booking the patients in for their required blood tests.
  • Monitoring the reports of any newly diagnosed or at risk (case finder) patients as per the clinical lead or Primary Care Contracts Manager
