This role is essential in supporting the Practice team to achieve outcomes from the Network Direct Enhanced Service (DES) Agreements which include:

  • Enhanced Health in Care Homes
  • Structured Medication Review and Medicines Optimization
  • Supporting Early Cancer Diagnosis
  • Anticipatory / Personalized Care

Key Responsibilities

This position necessitates a solid understanding of SystmOne to set up and run searches to identify specific patient groups, as directed by clinicians. Responsibilities include assisting with clinical audit, patient engagement, and fostering a positive service-user experience while removing access barriers for certain individuals.

Tasks involve administering digital health app dashboards, inviting patients for health checks, and contributing to quality improvement programs outlined in the DES.

Additional Job Responsibilities

The role necessitates proficiency in SystmOne to execute searches as directed by clinicians. Familiarity with Dorset Intelligence & Insight Service (DiiS) is also required, with training available if necessary.

  • Completing non-clinical Cancer Care Reviews via phone
  • Monitoring patients' cancer diagnostic testing requirements
  • Making Pharmrefer referrals to community pharmacies
  • Managing Spirometry waiting lists and appointments
  • Handling administrative tasks related to digital health apps

Tasks also include inviting patients for various health reviews, ensuring timely bookings, and supporting the team in achieving targets such as Quality Outcome Framework (QOF) and Investment and Impact Fund (IIF).

Responsibilities extend to managing NHS Digital Flags, facilitating communication adjustments, and supporting various administrative functions.

  • Booking structured medication reviews
  • Assisting with referrals to the First Contact Mental Health Nurses
  • Transcribing digital referrals and maintaining patient records

Additionally, the role involves encouraging patient engagement and seamless service experiences while supporting those facing barriers to access healthcare services.

To support network and practice teams with clinical audit and evaluation regarding DES outcomes and quality improvement initiatives.
