Who You Are

  • Educated to degree level with an appropriate professional qualification relevant to the role and specialism (e.g., CSS/CQSW or DipSw, Dip COT, or Degree in Occupational Therapy)
  • Registered with the HCPC
  • Knowledgeable about the Care Act 2014, the Mental Capacity Act 2005, and other relevant legislation
  • Demonstrates understanding of the principles and application of excellent adult social care
  • Capable of creating effective working relationships with internal and external customers
  • Possesses strong literacy, numeracy, interpersonal, and IT skills
  • Experienced in a similar role within a Local Authority is desirable
  • Experience with commissioning and coordinating delivery of services is a plus
  • Able to respond proactively to organizational change
  • Demonstrates a commitment to professional development


What the Job Involves

  • Manage a service caseload including customers with complex needs
  • Accept referrals, perform screenings, and carry out customer needs assessments
  • Apply eligibility criteria as identified in the Care Act to differentiate between essential and desirable customer needs
  • Provide professional supervision to unqualified staff
  • Maintain professional qualifications, memberships, and registrations
  • Develop and maintain effective working relationships with partners to deliver coordinated, timely services
  • Ensure compliance with professional codes of conduct and relevant legislation
  • Promote and engage in good team working practices
  • Provide effective communication within the team, across the council, and with partners
  • Deliver services and support according to agreed service standards
  • Contribute to the sharing of knowledge between teams and with external partners as appropriate
  • Undertake functional assessments for customers and carers, and commission necessary equipment
  • Recommending essential structural adaptations and liaising with professional bodies
  • Develop customer-focused medium and long-term support plans
  • Ensure safeguarding measures are appropriately actioned and progressed
