We are seeking an experienced Type 2 Diabetes Nurse Practitioner to join our client's nationwide Diabetes team. As a primary care professional, you will be responsible for delivering high-quality patient care and improving health outcomes for individuals with Type 2 diabetes.

In this role, you will work closely with patients to identify areas for improvement, develop individualised care plans, and provide ongoing support and education. You will also be involved in optimising oral hypoglycaemic treatments and referring patients for specialist insulin therapy when necessary.

To be successful in this position, you will require a strong understanding of diabetes management principles, excellent communication skills, and the ability to work effectively in a fast-paced environment. Salary: £45,000 per annum + benefits package. Working hours are Monday-Friday, 8:30am-5:30pm, with the option to consider part-time (4 days) arrangements.
